Telogen Effluvium — What It Is and Whether It Can Be Stopped in Warsaw

More hair on the brush, on the pillow, in the shower — and no bald patches, just a feeling that it is "leaving" from everywhere at the same time? Most likely, you are facing telogen effluvium. This is one of the most common types of hair loss and, at the same time, one of the least understood. The good news: in most cases, the process is reversible. But it is worth figuring out the cause.

What Is Telogen Effluvium?

To understand what is happening, you need to know one basic fact: hair lives in cycles. Each hair goes through three phases:

  • Phase 1 - AnagenActive growth lasts 2–6 years (approximately 85–90% of our hair).
  • Phase 2 - CatagenTransition phase — hair growth continues for a few weeks (approximately 1–3% of all hair).
  • Phase 3 - TelogenRest and shedding — lasts 2–4 months (makes up approximately 10–15% of all hair).

Normally, each follicle goes through these phases independently and at different times — thanks to this, we lose 50–100 hairs every day and do not even notice it. But under the influence of certain triggers — stress, illness, hormonal changes, deficiencies — a significant portion of follicles simultaneously "exits" anagen and transitions into telogen.

After 2–4 months following this shift, the follicles shed en masse. It is this condition that is called telogen effluvium — telogen effluvium. Understanding what telogen effluvium is removes the main fear: the follicles remain alive and capable of recovery. This does not mean the death of the hair, but only a disruption of its growth rhythm. The body, experiencing stress, simply redistributes its priorities.

It is important to distinguish between two courses of the condition: acute telogen effluvium — short-term, with a clear trigger, usually resolving after its elimination; and chronic — when the cause does not disappear or there are several of them simultaneously, and the process drags on for months and years. According to StatPearls (NCBI), telogen effluvium is one of the most common types of non-scarring alopecia in adults and is diagnosed significantly more often in women of reproductive age. But regardless of the form — without understanding the cause, treating such hair loss is difficult.

How to Recognize Telogen Effluvium

Telogen effluvium symptoms differ from other types of alopecia primarily by the nature of the shedding: hair does not fall out in patches, but evenly all over the head. More hair on the brush after combing, in the drain after a shower, on the pillow in the morning — this is the typical picture.

To understand how to recognize telogen effluvium more accurately, look at the hairs themselves: in telogen effluvium, they have a small white bulb at the end — a sign of a completed cycle. Another indicator is the pull test: if gently pulling a small bundle of hair results in 3 or more hairs coming out — this is already above normal. Diffuseness is also characteristic: thinning is even, without clear boundaries, most noticeable at the temples and the crown.

The Most Common Causes of Telogen Effluvium

Telogen effluvium causes always lie in a specific trigger. The most frequent of them are:

  • Acute or chronic stress.
  • Infections with a fever, COVID-19.
  • Pregnancy and childbirth.
  • Sharp hormonal changes.
  • Crash diets, sudden weight loss.
  • Deficiencies: ferritin, D, zinc, protein.
  • Surgical operations, anesthesia.
  • Chronic diseases.
  • Antidepressants, beta-blockers.
  • Isotretinoin and other medications.

A key feature of telogen hair shedding: it does not start immediately after the trigger, but after 6–12 weeks. It is this delay that complicates the search for the cause, because the person has already "forgotten" about the stressful event, while the hair is just beginning to react.

Telogen Effluvium After Stress, Illness, and Childbirth

The three most common problems for which people most often seek a consultation with a trichologist are: telogen effluvium after stress, telogen effluvium after illness, and telogen effluvium after pregnancy. The cause of all these problems is the same: the body experiences a load and redirects resources away from hair follicles to more critical systems.

After COVID-19, mass telogen effluvium was reported by millions of people. Journal Medicine and Life (2022) recorded diffuse alopecia as one of the most common cutaneous consequences of COVID-19. During pregnancy, elevated estrogen extends anagen — hair is thicker. After childbirth, hormones drop sharply, and follicles transition into telogen en masse. That is why telogen effluvium treatment consists primarily of finding and eliminating the root cause, rather than using symptomatic remedies from the pharmacy.

  • iron deficiency;

In such cases, it is important not to self-medicate, but to undergo a full diagnosis by a specialist.

How to Distinguish Telogen Effluvium from Other Types?

Not every intense shedding is telogen. Here are the key differences:

Are you wondering what to do for telogen effluvium in everyday life? First of all, this is about supporting recovery, not a cure. A few basic principles:

What you should do:

  • Mild sulfate-free shampoo.
  • Sufficient protein and micronutrients.
  • Less blow-drying and thermal tools.
  • Full sleep.
  • Photo-monitoring every month.
  • Consulting a specialist.

What you should not do:

  • Test all products at once.
  • Take supplements without tests.
  • Aggressively massage the head.
  • Wait for months for the problem to go away on its own.
